This means you can work with our medical providers via phone, zoom, email, or through the patient portal, order blood work to be done at a LabCorp facility near you, and have your medications shipped directly to your house.

All patients start with a complimentary consultation.  We recommend having blood work to review at your initial consultation.  If you have lab work showing a full hormone panel within the last 90 days, please email that to labs@elevatemd.com and that will be used at your appointment.  If not, we will order that through LabCorp for $550.

If you decide at your initial appointment that you would like to work with our office, the cost for our most common standard protocol is $2100 for the year. Patients may pay the full amount upfront or break that down into two payments of $1050 or 4 payments of $525.

Included:

  • Appointments with Medical staff every 3 months to review your progress, symptoms, blood work, and adjust your individual treatment plan
  • Reasonable communication via phone, email, and the patient portal
  • Follow up blood work including CBC, CMP, Total Testosterone, Free Testosterone, and  Estradiol up to 4 times yearly.
  • Medications and supplies including Injectable testosterone, insulin syringes, and shipping

IMPORTANT!!! Refills are included but are not automatically shipped

Additional:

  • Additional appointments above and beyond every 3 months
  • Other medications such as hCG or Tadalafil (Cialis)
  • Peptides
  • Additional bloodwork as recommended by your provider

Medications should arrive within 2 weeks. Your initial follow up appointment will be in 8-12 weeks with labs being ordered and drawn 2 weeks prior.  Going forward, appointments will be scheduled every 3-6 months.

Initial Bloodwork: CBC, CMP, Total Testosterone, Free Testosterone, Estradiol, Luteinizing Hormone, Sex Hormone Binding Globulin, Prostate Specific Antigen, Prolactin, and Vitamin D, 25OH as well as a full thyroid panel including Thyroid Stimulating Hormone, Free T3, Free T4, Reverse T3, Thyroperoxidase Antibody, and Anti-Thyroglobulin Antibody.

Our blood work pricing is exclusive for patients of Elevate MD.

We will send you a lab order that you will take to a LabCorp near you.

There are no locked in contracts, you just pay for the 3 months of medications and services that you request.

We do not accept insurance or provide assistance with the use of insurance.  We do provide an invoice that you can submit to your insurance company for the possibility of reimbursement. Click here to understand why.
